- A properly filled application form obtained from PHIPT site or online.
- 2 recent passport sized photographs.
- Photocopy of National Identity card.
- Photocopy of Birth Certificate
- Photocopy of GCE Advance Level Certificate or Baccalaureate or equivalent.
- For ND programs; photocopy GCE Ordinary Level Certificate or CAP or equivalent.
- For degree students only: a copy of HND results transcript or its equivalent.
- For Master's students: Certified copy of Bachelor's degree (BSc / B.Tech / HND equivalent), official academic transcripts, updated CV, and letter of motivation.
